In the client interest to sustain therapeutic constancy I require two months notice to cancel a therapy session. In the event you can provide at least 48 hour notice I will try my best to offer an alternative appointment within the next four days 

Drug / Alcohol Use

You need to abstain from using any non-prescribed drugs and alcohol on the day of your appointment, otherwise I will be unable to offer you a session.
